This is a remote position, allowing you to work from home while engaging with clients and their families through virtual platforms and telehealth solutions. Although the majority of your work will be conducted from a remote setting, you will still be expected to maintain flexibility in your schedule to accommodate client needs. Regular work hours are Monday to Friday, from 8:00 AM to 4:30 PM, with occasional evening or weekend availability for urgent cases. As communication is key in this role, you will utilize electronic health record (EHR) systems to document client interactions, track progress, and collaborate with interdisciplinary teams.

• Conduct comprehensive assessments of clients' psychosocial needs, evaluating emotional, mental, and social factors that influence their well-being.
• Work closely with clients and their families to develop individualized care plans that address specific goals, preferences, and life circumstances.
• Provide empathetic counseling and emotional support to seniors, assisting them in navigating significant life transitions, such as end-of-life adjustments.
• Advocate for clients' access to community resources and services, ensuring they receive the appropriate support necessary for their unique situations.
• Collaborate with interdisciplinary teams consisting of nurses, caregivers, and other healthcare professionals to enhance overall patient care.
• Regularly monitor clients through virtual visits, assessing their emotional and physical conditions and addressing any safety concerns or issues.
• Facilitate effective communication among family members involved in the care of senior clients, resolving conflicts and addressing any barriers to support.
• Educate families on available community resources, support groups, and programs designed to foster social engagement and improve overall quality of life.
• Maintain meticulous documentation of client assessments, care plans, progress notes, and relevant interactions in compliance with health regulations.
• Stay informed about best practices in social work, current policies, and regulations to uphold ethical standards in your practice.

In this role, you will primarily work with senior clients, many of whom are facing life-limiting illnesses and emotional challenges. • A Master's degree in Social Work from an accredited institution.
• Valid and active state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of experience in a social work capacity, ideally within a geriatric care or community health setting.
• Extensive knowledge of social work principles, particularly in relation to aging and end-of-life care.
• Demonstrated proficiency in assessment, counseling, and solution-oriented problem-solving.
• Exemplary ability to empathize with clients, establishing a supportive rapport with seniors and their families.
• Strong communication and interpersonal skills, fostering productive collaboration in interdisciplinary teams.
• Proficiency in using electronic health record (EHR) systems and relevant software applications.

• Experience working in hospice or palliative care settings is advantageous.
• Familiarity with community resources and support networks for seniors and their families.
• Knowledge of mental health treatment modalities for older adults is preferred.
