Work Environment

This is a remote position, allowing you to work from home while engaging with clients and their families through virtual platforms and telehealth solutions. Although the majority of your work will be conducted from a remote setting, you will still be expected to maintain flexibility in your schedule to accommodate client needs. Regular work hours are Monday to Friday, from 8:00 AM to 4:30 PM, with occasional evening or weekend availability for urgent cases. As communication is key in this role, you will utilize electronic health record (EHR) systems to document client interactions, track progress, and collaborate with interdisciplinary teams.

Key Responsibilities
  • Conduct comprehensive assessments of clients' psychosocial needs, evaluating emotional, mental, and social factors that influence their well-being.
  • Work closely with clients and their families to develop individualized care plans that address specific goals, preferences, and life circumstances.
  • Provide empathetic counseling and emotional support to seniors, assisting them in navigating significant life transitions, such as end-of-life adjustments.
