• Conduct comprehensive assessments of clients' psychosocial needs, focusing on their emotional, mental, and social well-being.
• Collaborate actively with clients and their families to create personalized care plans that align with specific life goals and preferences.
• Provide counseling and emotional support, guiding seniors and their families through significant life changes and challenges.
• Advocate on behalf of clients to ensure they have access to necessary community resources and services.
• Engage with interdisciplinary teams—including nurses and healthcare professionals—to coordinate care efforts and enhance client services.
• Conduct regular virtual check-ins to monitor client status, reassess their living situations, and address any concerns that may arise.
• Facilitate communication between family members regarding care options, ensuring everyone involved is on the same page.
• Educate clients on community resources, support groups, and wellness programs that contribute to their social engagement and improved quality of life.
• Document all client interactions, assessments, care plans, and progress accurately using electronic health record (EHR) systems.
• Stay updated on current social work practices and local regulations to maintain compliance with ethical standards.

Your primary focus will be on geriatric clients facing complex emotional and social challenges, including anxiety, depression, and significant life transitions. You will develop meaningful relationships with your clients and their families, ensuring they receive quality support in navigating their unique circumstances.

• A Master's degree in Social Work from an accredited institution.
• Active and unrestricted state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of professional experience in social work, preferably within a community health or geriatric-focused environment.
• Comprehensive understanding of social work principles, gerontology, and community resources available to seniors.
• Exceptional assessment, counseling, and conflict-resolution skills.
• Proven ability to genuinely empathize, establish trust, and communicate openly with older adults and their families.
• Strong collaborative spirit to work effectively with multidisciplinary teams.
• Competence in electronic health record (EHR) systems and standard software applications.

Experience working with diverse populations and in a community health context is highly desirable. Familiarity with local social service agencies and programs that serve older adults will be an added advantage. Knowledge of evidence-based practices in mental health will also contribute to your effectiveness in this role.
