What You'll Do

  • Conduct person-centered assessments to identify each participant's needs, preferences, and life goals.
  • Develop, implement, and regularly update Individual Support Plans (ISP) to ensure participants receive the right services.
  • Provide participants with choice and advocacy, helping them make informed decisions about providers and care options.
  • Coordinate with medical professionals, behavioral specialists, family members, and community resources to deliver seamless, high-quality care.
  • Monitor and evaluate services to ensure participants' safety, satisfaction, and progress toward independence.
  • Support both individuals with disabilities and members of the aging population in accessing the community resources they need.
  • Maintain documentation, confidentiality, and compliance with all program and ethical standards.
  • Participate in ongoing training and professional development to strengthen your skills and expand your impact.

What You Bring

  • Bachelor's degree in social work, Psychology, Human Services, or related field.
  • Strong organizational, communication, and advocacy skills.
  • Comfort working independently and traveling (up to 50%) to meet with participants in their homes and communities.
