As a Licensed Clinical Social Worker at Franklin Health System, your responsibilities will include:

• Conducting thorough assessments to understand the social, emotional, and environmental challenges faced by clients.
• Designing and implementing individualized service plans that set clear goals, specify interventions, and monitor progress over time.
• Providing compassionate counseling and emotional support to individuals and families navigating through personal crises and transitions.
• Collaborating with clients to create personalized service plans, enhancing their access to essential community resources.
• Offering timely crisis intervention services, conducting risk assessments, and formulating safety plans when necessary.
• Working closely with local community agencies and organizations to ensure seamless coordination of services and effective advocacy on behalf of clients.
• Engaging in community outreach initiatives aimed at raising awareness and promoting the full spectrum of our agency services.
• Participating in case conferences, team meetings, and ongoing professional development opportunities to enhance service delivery.

In this role, you will primarily work with a diverse client base, including individuals and families facing significant life challenges such as mental health issues, economic hardships, and social isolation. Your objective will be to empower clients to navigate these challenges and improve their overall wellbeing, ensuring they have the support they need to thrive.

The ideal candidate will possess the following qualifications:

• A Master's degree in Social Work (MSW) from an accredited program.
• Current and active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Arizona.
• Strong understanding of social and economic justice issues and a commitment to addressing these matters in practice.
• Exceptional communication and interpersonal skills, with an ability to interact effectively across diverse cultural backgrounds.
• Problem-solving abilities coupled with a compassionate and client-centered approach.

While not mandatory, preference will be given to candidates with:

• Experience working in a social service agency or a similar environment, particularly within hospice or palliative care settings.
• Knowledge of trauma-informed care principles and community resource navigation.
• Demonstrated history of engaging in effective community outreach and service coordination.
