Key Responsibilities

• Conduct comprehensive assessments to evaluate the social, emotional, and environmental challenges faced by clients.
• Develop individualized service plans that include specific goals, interventions, and methods for progress monitoring.
• Provide tailored counseling and support to individuals and families navigating life transitions or crises.
• Engage collaboratively with clients to create personalized service plans, linking them to vital community resources.
• Deliver crisis intervention services, performing risk assessments and implementing safety planning as needed.
• Actively collaborate with community agencies to coordinate services and advocate for client needs effectively.
• Participate in community outreach efforts aimed at raising awareness of crucial social issues and promoting Franklin Health System’s services.
• Join case conferences, team meetings, and partake in ongoing staff training to continuously improve service delivery and professional development.

Patient Population / Client Focus

The Licensed Clinical Social Worker will engage with a diverse range of clients, including individuals and families from various socio-economic backgrounds. You'll be working with those experiencing critical life challenges—such as mental health crises, substance abuse issues, and the effects of poverty—helping them identify and access necessary resources to foster resilience and empower them towards a healthier future.

Qualifications

• A Master’s Degree in Social Work from an accredited program is required.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• Strong understanding of social and economic justice issues, as well as community support resources.
• Excellent communication, cultural awareness, and problem-solving skills are essential for effectively engaging with clients.

Preferred Experience

• Prior experience in a social service agency or similar environment is an asset.
• Familiarity with trauma-informed care and evidence-based practices enhances your effectiveness in providing support.

Compensation and Benefits
