Work Environment

This position is fully remote, allowing flexibility while you engage with clients across various locations. Our Hospice Center serves as a central hub for resource sharing and collaboration among staff members. Key Responsibilities
  • Provide one-on-one and group therapy sessions to clients struggling with major mental illnesses and severe emotional disorders, using evidence-based practices.
  • Conduct comprehensive client assessments, develop individualized treatment plans, and evaluate progress to ensure effective interventions are in place.
  • Facilitate crisis intervention services when necessary and collaborate with other health care providers to deliver integrated care.
  • Participate actively in team meetings, contributing insights and strategies to advance overall client care and service delivery.
  • Maintain thorough documentation of all interactions and treatment progress in compliance with organizational policies and professional ethical standards.
  • Engage in ongoing training and development opportunities to enhance clinical skills and stay current with best practices in mental health.
  • Contribute to quality improvement initiatives by participating in committees aimed at enhancing service delivery and client outcomes.
Patient Population / Client Focus

This role primarily serves individuals from diverse backgrounds who experience a range of mental health challenges, including anxiety, depression, trauma, and other severe emotional disorders. Providers will work closely with clients, their families, and community resources to ensure comprehensive support, facilitating access to vital services and promoting empowerment through client-centered care. By addressing the unique needs of the populations we serve, we can significantly enhance overall community health outcomes.

Qualifications
  • A Master’s degree in Social Work from an accredited institution, with current LCSW licensure in Arizona.
  • Minimum of 2 years of direct clinical experience in a mental health setting, demonstrating expert knowledge in assessment, diagnosis, and treatment of mental health disorders.
  • Exceptional verbal and written communication skills, capable of conveying complex information clearly and compassionately.
  • Strong organizational and planning skills to manage caseloads effectively.
  • Ability to work collaboratively with a multidisciplinary team and engage with diverse populations effectively.
Preferred Experience
  • Experience working in hospice or palliative care settings is highly valued.
  • Familiarity with electronic health record systems and telehealth practices.
  • Prior involvement in community outreach programs or services that address health disparities.
Compensation and Benefits

The salary for this position ranges from $70,000 to $85,000 annually, commensurate with experience.
