The typical working hours are Monday through Friday, from 9:00 AM to 5:00 PM, making it easier to balance work and personal life.

• Establishing effective communication as a liaison with families, outpatient providers, and community resources to ensure holistic support for clients.
• Engaging with insurance companies for pre-certification and utilization management to streamline client access to necessary services.
• Contributing actively to discharge and aftercare planning, enhancing the continuity of care for clients as they transition through different levels of support.
• Providing therapeutic meal support, promoting a healthy relationship with food for clients undergoing dietary challenges.
• Delivering empathetic, affirming care that addresses the intersections of clients' identities, ensuring they receive customized support tailored to their backgrounds and experiences.

This position primarily focuses on older adults who may face complex mental health challenges. Our LCSWs are entrusted with addressing a spectrum of mental health needs, from cognitive impairments to emotional distress, fostering resilience and independence in our clients.

• A Master's degree in Social Work from an accredited institution is required.
• A valid LCSW license in the state of Tennessee is a must.
• Prior experience working with older adults is essential to effectively meet the needs of our client population.
• Strong clinical and diagnostic skills, enabling accurate assessments and the development of targeted treatment plans.
• Excellent communication and interpersonal skills to foster positive working relationships with clients, families, and stakeholders.
• Ability to work both independently and collaboratively within a team, demonstrating initiative while valuing shared input.

• Experience with evidence-based therapeutic modalities, particularly those beneficial to older adults.
• Familiarity with electronic health record (EHR) systems to maintain accurate client documentation and streamline communication.
• Demonstrated ability in crisis intervention and management, providing timely support as needed.
