Freedom Psychotherapy Services is seeking a dedicated and Licensed Clinical Social Worker (LCSW) to join our remote team specializing in hospice care. In this full-time position, you will utilize your skills to support seniors and their families, providing crucial emotional and psychosocial assistance as they navigate the complexities of end-of-life issues. Work Environment

This remote position allows for flexibility while providing support to our hospice center clients. You will primarily work during standard business hours, Monday through Friday, from 8:00 AM to 4:30 PM, with occasional evening or weekend hours as necessary to meet client needs. The remote setting requires strong self-management skills and the ability to use telehealth platforms effectively to conduct assessments, provide therapy sessions, and maintain communication with clients and families.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments to identify the emotional, mental, and social needs of seniors in hospice care.
  • Collaborate closely with clients and their families to create personalized care plans that align with their unique goals and wishes.
  • Provide counseling services, offering emotional support to clients and assisting them in coping with transitions, grief, and end-of-life issues.
  • Advocate for clients, ensuring they have access to necessary resources and services within the community to support their well-being.
  • Document all interactions systematically using our electronic health record system, ensuring compliance with relevant regulatory standards.
Patient Population / Client Focus

In this role, you will primarily support senior clients receiving hospice care. Your work will directly influence their quality of life during a sensitive and critical time. You will also engage with families, addressing their concerns and helping them navigate the emotional complexities associated with hospice services. By fostering relationships built on trust and understanding, you will provide invaluable support to families as they process their experiences.

Qualifications
  • A Master's degree in Social Work from an accredited institution is required for this position.
  • Current and valid LCSW licensure in the state of Nevada is mandatory.
  • At least two years of experience in a social work role, particularly within a geriatric or hospice context.
  • Strong foundation in social work principles, aging-related issues, and knowledge of community resources available for seniors.
  • Exceptional interpersonal, assessment, and problem-solving skills.
Preferred Experience
  • Experience working within a hospice or palliative care team setting.
