Nurse Practitioner - Primary Care physician job in Minnesota :

A health services organization in Minnesota is seeking a licensed Nurse Practitioner (NP) for a Remote Patient Monitoring (RPM) role. This position involves providing telemedicine-based clinical support for patients enrolled in an RPM program, including new patient visits, ongoing monitoring, and patient education.

About the Opportunity:

  • Shift: Day shift
  • Schedule: Monday to Friday (approximately 3 days per week)
  • Hours: Full 8-hour shifts
  • Setting: Remote

Responsibilities:

  • Conduct telemedicine visits with new patients enrolling in the RPM program
  • Review patient demographics, medication history, current diagnoses, and home-monitoring data
  • Establish baseline monitoring parameters and educate patients on program expectations
  • Monitor RPM alerts, review follow-up needs, and address patient questions related to care
  • Document encounters in the telemedicine and RPM platform and escalate clinical concerns when appropriate

Qualifications:

  • Active and unrestricted Nurse Practitioner (NP) license in Minnesota
  • Experience managing patients with chronic conditions such as congestive heart failure, COPD, and diabetes
