• Conduct comprehensive clinical assessments, developing individualized treatment plans that reflect evidence-based practices and solution-oriented strategies.
• Provide therapeutic interventions that are tailored to the distinct mental health needs of our patients, employing modalities such as cognitive behavioral therapy (CBT) and mindfulness-based approaches.
• Document clinical progress and maintain a high standard of compliance by keeping accurate, timely clinical records in the designated electronic health record systems.
• Collaborate seamlessly with a multidisciplinary team, incorporating feedback and insights from medical professionals, nursing staff, and other social workers to ensure coordinated and holistic care.
• Engage in case consultations and participate in regular peer review meetings to uphold and enhance the quality of care provided.
• Educate and guide patients and their families, helping them navigate the complexities of mental health challenges, fostering empowerment through knowledge and resources.

In this role, you will work primarily with patients in hospice care, a demographic that requires sensitivity, understanding, and a proactive approach to mental health. • A Master's degree in Social Work from an accredited institution is mandatory.
• A valid LCSW license in the state of Pennsylvania is required.

• Annual salary ranging from USD 80,000 to 100,000, commensurate with experience.
