As a Licensed Clinical Social Worker, your key responsibilities will include:

• Conducting comprehensive assessments to identify patient and family needs while performing intensive counseling, psychotherapy, and case management services.
• Completing thorough mental status examinations and monitoring changes in clients' psychological conditions to adjust care strategies effectively.
• Maintaining detailed, accurate case notes, progress reports, and individualized treatment plans in compliance with the organization’s standards and HIPAA regulations.
• Utilizing a calm and empathetic approach to de-escalate difficult or high-stress situations, providing support that alleviates emotional distress.
• Collaborating closely with other healthcare professionals including psychiatrists, nurses, and community service providers to create a seamless continuum of care.
• Facilitating both individual and group therapeutic interventions tailored to clients' specific needs, helping them process their experiences and emotions.
• Sharing insights and therapeutic strategies with colleagues to enhance team effectiveness and the overall quality of care.

You will primarily work with clients in the hospice setting, focusing on individuals and families dealing with terminal diagnoses and end-of-life issues. This population often exhibits a wide range of emotional responses, including anxiety, depression, and complicated grief. Your role will involve guiding them through these challenging experiences, utilizing compassionate support to promote psychological resilience and healing.

The ideal candidate will have:

• A Master’s degree in Social Work from an accredited program.
• An active California license (LCSW) with the ability to practice independently in a clinical capacity.
• A high level of emotional intelligence, empathy, and the ability to build rapport and trust with clients and families.
• Demonstrated proficiency in various therapeutic modalities, and the adaptability to employ these techniques effectively in diverse settings.

While not mandatory, candidates with the following experiences will be preferred:

• A minimum of two years of clinical experience in a healthcare setting, especially in hospice or palliative care.
• Experience working with grief and loss, as well as advanced knowledge of medical and psychosocial dynamics involved in end-of-life care.
• Familiarity with electronic health record (EHR) systems is a plus.
