As a Licensed Clinical Social Worker at Serene Sanctuary Counseling, LLC, your responsibilities will include:

• Conducting comprehensive mental health assessments, including psychosocial evaluations, to identify client needs and support systems.
• Collaborating with patients to develop individualized treatment plans that are tailored to their goals and cultural backgrounds.
• Providing evidence-based therapeutic interventions, such as cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), and solution-focused brief therapy (SFBT) in various formats including individual, couple, family, and group settings.
• Offering crisis intervention services, ensuring immediate support and resources for patients experiencing acute distress.
• Maintaining meticulous clinical documentation, including progress notes and treatment plans, in accordance with regulatory guidelines and clinic policies.
• Empowering patients and their families through psychoeducation, equipping them with skills to effectively manage their mental health conditions.
• Collaborating with a diverse multidisciplinary team—comprising psychiatrists, psychologists, nurses, and case managers—to coordinate comprehensive and continuous care for patients.
• Conducting ongoing outcome assessments to evaluate the effectiveness of therapeutic interventions and adjusting treatment plans as needed.
• Keeping abreast of current research and best practices in mental health to enhance the quality and efficacy of therapeutic practices.
• Adhering to ethical guidelines and ensuring the confidentiality of sensitive patient information.

Your work will primarily focus on patients in hospice care and their families who are dealing with emotional and psychological challenges related to end-of-life issues. This population often requires a unique approach that incorporates grief counseling, bereavement support, and palliative care considerations. You will be instrumental in providing emotional support during a pivotal and sensitive time in their lives.

The ideal candidate will possess the following qualifications:

• A Master’s degree in Social Work from an accredited institution is required.
• An active State license as a Licensed Clinical Social Worker (LCSW).
• Demonstrated ability to work both independently and collaboratively within a multidisciplinary team setting.
• Strong interpersonal and communication skills, enabling the building of rapport with clients from various cultural backgrounds.

While not mandatory, preference will be given to candidates with experience in hospice care, palliative care, or working with terminally ill patients. Familiarity with relevant electronic health records (EHR) systems and therapeutic modalities specific to end-of-life counseling will be advantageous.

Serene Sanctuary Counseling, LLC offers an attractive salary range of $115,000 to $130,000 annually. We recognize that compensation can vary based on geographic location, experience, education, and individual skills. Our benefits package includes:
