This position is a full-time role based in a remote work setting, providing flexibility and work-life balance. While your day-to-day responsibilities will be carried out from the comfort of your own home, you will remain closely connected with our multidisciplinary team, collaborating seamlessly via telehealth platforms and virtual meetings to discuss patient progress, treatment plans, and care coordination. • Conduct thorough psychosocial assessments to understand patients' mental health status, strengths, and needs, ensuring individualized care approaches.
• Deliver evidence-based therapies, including cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), tailored to meet the unique circumstances of patients facing terminal illness.
• Collaborate with psychiatrists, nurses, and other healthcare professionals to design cohesive care strategies that promote overall patient well-being.
• Provide education and support for patients and their families regarding diagnoses, treatment options, and the complexities of medications involved in end-of-life care.
• Facilitate individual, group, and family therapy sessions using evidence-based modalities to support emotional healing and coping strategies.
• Collaborate with the treatment team to develop comprehensive treatment plans that integrate patient preferences and goals into care initiatives.
• Assist patients in navigating the mental health system, advocating for their rights and needs throughout their healthcare journey.
• Organize and lead therapeutic groups, psychoeducation sessions, and discharge planning meetings, ensuring every patient's journey is as supportive and structured as possible.

The patients served in our hospice center include individuals facing terminal illnesses and their families, requiring a gentler approach to mental health care. • A Master's degree in Social Work from an accredited program is required.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) is essential.

• Ability to function effectively as part of a multidisciplinary team, demonstrating collaboration and communication skills.

• Prior experience in hospice or palliative care settings.
• Exposure to evidence-based therapeutic practices, particularly in group therapy settings.
• Experience in navigating complex mental health systems and providing advocacy for vulnerable populations.
