• Conduct in-depth assessments to evaluate clients' mental health needs and develop customized treatment plans aimed at promoting positive outcomes.
• Implement a range of evidence-based therapeutic interventions, such as cognitive-behavioral therapy (CBT) and mindfulness strategies, to help clients manage their symptoms and cope with their experiences.
• Maintain meticulous clinical documentation that complies with HIPAA regulations, ensuring client confidentiality and data integrity at all times.
• Regularly engage with a multidisciplinary team, including nurses, physicians, and other therapists, to create a cohesive care strategy that addresses all aspects of the client's well-being.
• Provide psychoeducation and support to clients and their families, guiding them through decision-making processes regarding mental health treatment and resource allocation.

In this role, you will primarily serve a diverse patient population within a hospice setting, focusing on individuals facing end-of-life issues. This requires a nuanced understanding of grief, loss, and the emotional complexities associated with terminal illness. • A Master’s degree in Social Work from an accredited institution.
• An active LCSW license issued by the state of California.
• A minimum of 2 years of experience in clinical practice, ideally within a hospice or palliative care context.
• Demonstrated proficiency in trauma-informed care and solution-focused therapy methods.
• Excellent communication and interpersonal skills, enabling effective interactions with clients, families, and multidisciplinary team members.
• Strong analytical and critical thinking abilities to assess situations and develop effective solutions.

Experience in a hospice or healthcare setting is highly desirable, as is familiarity with tools and methodologies specific to end-of-life care. Background in crisis intervention and bereavement counseling will enhance your ability to support clients through the emotional challenges they face.
