JOB OVERVIEW
As a Clinical Adherence Nurse (RN), you will play a key role in enrolling, engaging, and supporting patients in a high-impact medication adherence program designed to improve health outcomes. This innovative role uses asynchronous video communication and digital engagement to build meaningful patient relationships and deliver personalized clinical support.
Through daily video check-ins, you will connect with patients, offering encouragement, education, and accountability that helps them stay on track with their medications and care plans. Using your clinical expertise and motivational communication skills, you will guide patients through challenges, celebrate progress, and help them build sustainable health habits.
In this role, you will help patients overcome barriers related to social determinants of health, provide tailored education, and monitor medication-taking behavior through video-based interactions. By building trust through engaging video communication, you will inspire patients to actively participate in their care and empower them to take greater control of their health.
Key Responsibilities:
  • Conduct patient outreach and engagement through video messaging, video consultations, phone calls, and text-based communication, educating and enrolling patients into the adherence program
  • Utilize video-based communication as a primary engagement tool to deliver personalized education, coaching, and adherence support
  • Provide compassionate and motivational coaching, case management, and health education services to encourage medication adherence and lifestyle changes
  • Conduct video-based medication reconciliation consultations, ensuring patients understand their medications, dosing schedules, and proper administration techniques
  • Review asynchronous video submissions of patients taking medication, ensuring correct administration, monitoring adherence, and assess for side effects or concerns
  • Create short-form educational and motivational videos to send to patients for personalized engagement and population-level health education
  • Assess patient needs, including medical, behavioral, and social challenges, and escalate cases as necessary.
  • Provide guidance on medication administration, inhaler technique, chronic disease management, and medication safety through live and asynchronous video coaching
  • Act as a clinical escalation point for non-clinical team members to send patient cases that need a higher level of clinical review and intervention
  • Assist patients with technical troubleshooting related to the company's mobile application and adherence platform
  • Maintain accurate and detailed documentation of all interactions, ensuring compliance with company and regulatory guidelines
  • Engage with provider offices, health plan services, and pharmacies on behalf of the patient, as needed, to overcome barriers
  • Provide referrals to healthcare providers and community resources when needed, including direct outreach to provider offices and/or warm transfers to service providers
  • Utilize motivational interviewing techniques within video and digital engagement to engage patients in long-term adherence strategies
  • Offer guidance on inhaler techniques, chronic disease management, and medication safety
  • Contribute to program improvement by providing insights from patient interactions, particularly regarding video engagement, digital adherence monitoring, and patient education strategies
  • Partner with cross-functional teams to develop and refine video-based patient engagement content, clinical protocols, and educational materials

Required Qualifications:
  • Bilingual in Spanish
  • Registered Nurse (RN) license in good standing with at least 5 years of nursing experience
  • 5 years of related experience in patient engagement, health education, or care coordination
  • Strong background in chronic disease management, medication adherence, and patient coaching
  • Excellent oral and written communication skills with a warm, empathetic approach
  • Ability to work in a fast-paced, patient-focused environment
  • Proficiency in electronic health records (EHR), mobile applications, and digital health platforms
  • Comfortable engaging patients via phone, video, and chat throughout the workday
  • Experience assessing social determinants of health and connecting patients with resources

Preferred Qualifications:
  • Experience in video communication, telehealth, community health work, or digital health settings
  • Certification in health coaching (e.g., Certified Health Coach, Integrative Health Coach, etc.) or willingness to obtain certification within a specified timeframe
  • Knowledge of Directly Observed Therapy (DOT) models
  • Experience working with Medicaid and Medicare Dual Eligible (D-SNP) populations
  • Familiarity with Salesforce or other CRM platforms
