This remote position allows you to work from the comfort of your home while connecting with patients via telehealth platforms. As part of the Federally Qualified Health Center (FQHC) system, you'll be an essential part of a collaborative primary care team. • Collaborate with primary care practitioners to assess and manage mental health concerns, playing a pivotal role in the overall wellness of patients.
• Conduct thorough patient intakes emphasizing diagnostic assessments, developing treatment plans tailored to individual needs.
• Deliver targeted interventions to support patients facing mental health challenges, advising on best practices for therapeutic engagement.
• Maintain regular communication with primary care providers to ensure continuity of care and adapt treatment strategies based on patient feedback and progress.
• Monitor patient recovery, initiating follow-up procedures to evaluate the effectiveness of therapeutic techniques and adjust as necessary.
• Identify at-risk patients and develop preventative strategies to enhance their psychological resilience.
• Supervise the delivery of behavioral health services, ensuring synergistic operations between various healthcare providers.
• Document patient progress meticulously, ensuring all treatment records reflect accurate and meaningful information.
• Educate patients about self-management, equipping them with the skills necessary for proactive engagement in their mental health journey.
• Abide by the Code of Ethics for Clinical Professionals, ensuring the highest standard of care in every interaction.

In this role, you will work with a diverse range of patients, including those dealing with anxiety, depression, and various behavioral disorders. Understanding the personal and social implications of mental health issues is crucial as you support individuals from diverse backgrounds to navigate their emotional challenges and lead healthier lives.

• Master’s Degree in Clinical Counseling or a related field is required.
• A current and valid Licensed Professional Counselor (LPC) license is essential.
• Comprehensive knowledge of mental health assessment tools, treatment planning methodologies, and psychotherapeutic techniques.
• A strong grasp of the DSM-IV diagnoses, as well as an understanding of behavioral therapy approaches.
• Capacity to work effectively within a multidisciplinary team, communicating patient needs and treatment progress effectively.
• Physical and mental well-being sufficient to meet job demands, along with the ability to engage and motivate individuals in a telehealth environment.

• Bi-lingual proficiency in English and Spanish is highly preferred to broaden accessibility for our diverse patient population.
• Previous experience in a telehealth setting or with Federally Qualified Health Centers is a plus.

Mind and Body Connect Wellness Center offers a competitive salary range of USD 80,000 to 100,000 annually, commensurate with experience and qualifications.
