About Home Care Network

Provide compassionate ADL/personal care (bathing, grooming, dressing, toileting, mobility/transfer support)

Perform vital signs and observe/report changes in condition

Support safe ambulation and basic home exercises per the care plan

Light homemaking related to the plan of care (linen changes, simple meal prep, patient-area tidying)

Document visits clearly and on time (mobile/app comfort a plus)

Collaborate with the RN and clinical team to deliver excellent outcomes

Schedule & Territory

PRN, weekday daytime visits (set your availability)

Assignments clustered within a reasonable radius when possible (Rockwall North Dallas Richardson/Plano)

Pay & Perks

Flexible PRN scheduling that works with your life

Supportive, RN-led team and quick, friendly onboarding

Meaningful one-to-one patient timeโ€”see your impact immediately

Valid Texas driverโ€™s license, reliable vehicle, and auto insurance (Required)

1+ year CNA experience (Required); Home Health experience preferred

Ability to lift/assist up to 50 lbs and safely perform transfers

Professional, reliable, and compassionate bedside manner

Basic comfort with mobile documentation (training available)

Weโ€™d love to meet you and share why caregivers chooseโ€”and stay withโ€”HCN.

HCN is an equal opportunity employer and welcomes all qualified applicants.
