You will collaborate with clinical directors and multi-disciplinary teams through telehealth platforms, enhancing communication and support for your client caseload. Our Hospice Center provides a framework of resources and collaborative efforts designed to facilitate recovery support tailored to individual needs.

As an LCSW at West Valley Psychiatric Services, your primary responsibilities will include:

• Conducting thorough assessments to determine the counseling needs of clients, particularly in the areas of substance abuse and mental health.
• Utilizing the DSM (Diagnostic and Statistical Manual) criteria to assess clients for substance abuse diagnoses, ensuring that the appropriate level of care is established based on ASAM (American Society of Addiction Medicine) guidelines.
• Preparing client treatment plans that incorporate substance abuse counseling and are tailored to each individual's unique situation.
• Developing comprehensive discharge and continuum of care plans to facilitate smooth transitions for clients exiting treatment.
• Maintaining precise documentation of all counseling services, assessments, and treatment plans within the clients' clinical records.
• Managing a client caseload, which includes regular updates for case management and utilization reviews, treatment plans, and clinical charting.
• Coordinating support services for clients and families, recording all provided services in the clinical record.
• Engaging with external referral sources such as legal representatives, educational institutions, and employers to collaborate on client support strategies.
• Facilitating interventions for crisis-oriented issues by collaborating with a multi-disciplinary team to ensure clients receive comprehensive care.

Your clients will primarily include individuals struggling with substance use disorders and co-occurring mental health issues. You will play a crucial role in providing therapeutic support, engaging clients in individual and group therapy sessions, and fostering a recovery-oriented environment. Your contributions will directly influence their journey toward better mental health and substance recovery, reinforcing their capabilities to lead fulfilling lives.

To thrive in this role, candidates must possess:

• A valid Licensed Clinical Social Worker (LCSW) credential in the state of Colorado.
• A Bachelor's Degree in Social Work or a related field.
