Key Responsibilities
  • Conduct ongoing assessments that identify the specific needs of each patient, adapting therapeutic approaches as necessary.
  • Establish effective communication networks among supervisors, direct care staff, family members, and other supportive individuals to monitor and discuss each client's progress.
  • Maintain a clear tentative schedule for clients, providing supervisors with insights into appointment availability, home visits, and monitoring of intervention outcomes.
  • Coordinate and execute home visits for in-depth observations, which may include chart reviews and staff interviews, to ensure comprehensive understanding of client circumstances.
  • Implement program plans effectively, ensuring all therapeutic interventions are executed as designed and assessed regularly.
  • Actively participate in Interdisciplinary Team Meetings, ensuring that insights from interactions with clients contribute to collective treatment strategies.
  • Complete monthly and quarterly progress notes that document client growth and challenges, ensuring continuity of care.
  • Organize and facilitate training sessions to ensure that all staff and team members are proficient in the implementation of program plans and documentation requirements.
