This role is designed for a telehealth setting, allowing for flexibility and the ability to reach patients in the comfort of their own homes. • Conduct thorough patient intakes, focusing on diagnostic assessments and functional evaluations to develop personalized treatment plans.
• Provide brief, focused therapeutic interventions for patients requiring mental health services, ensuring adherence to established clinical guidelines.
• Collaborate with primary care providers to identify mental health needs, offering recommendations for treatment adjustments and specialty referrals when necessary.
• Develop and implement relapse prevention plans, monitoring patient progress and adjusting strategies as needed.
• Document all patient interactions and progress in treatment charts, maintaining compliance with all regulatory and ethical standards.
• Educate patients on self-management skills, empowering them to take charge of their treatment and recovery process.
• Assist in the identification of at-risk patients and create proactive plans to mitigate the escalation of mental health conditions.
• Participate in team meetings to provide insights on behavioral health services and contribute to program evaluation efforts.
• Offer support to patients in achieving compliance with any medical treatment prescribed by their primary care provider.
• Uphold the Code of Ethical Conduct for Clinical Professionals and engage in continuous professional development.

• A Master's degree in Clinical Counseling or related field is required.
• A current and valid LPC (Licensed Professional Counselor) license is mandatory.
• Proficient in mental health assessment techniques, treatment planning, and familiarity with DSM-IV diagnoses.
• Strong interpersonal and communication skills, essential for establishing rapport with patients and professionals alike.
• Ability to work independently while being a proactive member of a multidisciplinary care team.
• Thorough understanding of the social determinants of health and their impact on mental health outcomes.

• Experience with telehealth platforms and providing virtual counseling services.
• Bi-lingual proficiency in English and Spanish is highly desirable, enhancing our ability to serve a broader patient population.
• Background in family therapy or group therapy techniques is an advantage.
