• Conduct comprehensive patient intakes, focusing on diagnostic evaluations and functional assessments, making informed treatment recommendations to primary care providers.
• Engage in brief but focused therapeutic interventions for patients requiring mental health support, adhering to established therapy methodologies.
• Collaborate closely with primary care providers to recognize and address mental health and psychosocial issues among patients, acting as a valuable member of the interdisciplinary team.
• Monitor patient progress by conducting follow-up evaluations, adjusting treatment plans as necessary to ensure effective care.
• Assist in the development of relapse prevention plans for patients at risk, ensuring they maintain stable functioning and improved mental health.
• Document and maintain accurate treatment records, patient appointments, and progress notes, ensuring compliance with agency guidelines and regulations.
• Provide education and resources to empower patients with self-management skills, fostering their active participation in treatment.
• Offer consultations to clinic management regarding behavioral services and participate in program evaluation initiatives.
• Encourage healthier lifestyle choices by effecting behavioral changes in patients with physical disorders.
• Navigate community resources effectively to link patients with relevant support systems and services.
• Perform additional duties as assigned that contribute to the overall functioning and success of the clinic.

This role requires a strong focus on a diverse patient population, including individuals facing a range of mental health challenges. You will work with patients across different demographics, providing culturally competent care and sensitivity to their unique circumstances and backgrounds. Your interventions will aim to enhance their overall quality of life, making a tangible impact within the community.

• A minimum of a Master's Degree in Clinical Counseling from an accredited institution.
• Active and current LPC (Licensed Professional Counselor) License.
• Proficient understanding of mental health assessment, treatment planning, and modern therapeutic techniques including family therapy, group therapy, and various modalities of psychotherapy.
• Exceptional communication skills, capable of establishing rapport and maintaining effective relationships with patients, families, and team members.
• Ability to work autonomously in a remote environment while contributing to a team-oriented setting.

• Bi-lingual proficiency in English and Spanish is highly preferred, enhancing our ability to serve a wider patient demographic.
• Demonstrated knowledge of community resources and healthcare systems specific to behavioral health.
• Experience working in a primary healthcare setting or community health clinic is advantageous.
• Familiarity with electronic health record (EHR) systems for documentation and patient tracking.

The Core For Relationship Healing offers a competitive salary range of USD 87,000 – 105,000 annually, commensurate with experience and qualifications.
