Community Health Network (CHN) is seeking a dedicated and compassionate Full-Time Licensed Clinical Social Worker (LCSW) to join our dynamic team. As a remote position, this role offers the flexibility to work from your home office while maintaining a high level of collaboration with colleagues and community partners via digital communication platforms. You will participate in regular team meetings, virtual training sessions, and outreach activities, enhancing your skills while contributing meaningfully to program development. While the standard working hours are during typical business times, occasional evening and weekend hours may be required for special events and programs.

• Conduct thorough assessments to evaluate clients' social, emotional, and psychological needs, utilizing standardized assessment tools and personal interviews.
• Provide individualized counseling sessions and group therapy, employing evidence-based techniques tailored to specific client situations.
• Collaborate with clients to develop actionable service plans that address their goals and align with best practices in mental health care.
• Advocate for clients’ rights and access to community resources, connecting them with additional services such as housing, financial assistance, and health care.
• Actively contribute to program evaluation and grant writing initiatives, ensuring continuous improvement of services offered.
• Facilitate workshops and support groups that address pertinent social issues and promote community engagement.
• Maintain accurate and timely documentation of client interactions and progress in accordance with organizational policies and regulatory requirements.

Your primary focus will involve working with a diverse client population, including individuals and families facing challenges related to mental health, substance abuse, trauma, and socioeconomic factors. By providing culturally competent services and a trauma-informed care approach, you will help clients navigate their circumstances and achieve improved life outcomes.

• A Master’s degree in Social Work from an accredited program is required for this position.
• Possession of an active state licensure as a Licensed Clinical Social Worker (LCSW).
• Strong communication, empathy, and organizational skills aimed at fostering effective client relationships and collaboration with team members.
• Understanding and dedication to social justice principles and advocacy efforts within the community.

• Previous experience working within a nonprofit organization or similar settings is highly preferred.
• Familiarity with trauma-informed care practices, community resources, and social advocacy.
• Experience in working with clients from diverse backgrounds, with a particular focus on underserved populations.
