Job description

Looking for a change of scenery and a chance to help others. Join a traveling Physical Therapist role in Oak Brook, IL where you will help adult and geriatric patients recover from injuries and manage chronic conditions in a home health setting. This position involves patient visits across a designated coverage area with virtual meetings and remote support. Orientation will be onsite in Oak Brook for approximately two days followed by remote work within the coverage area.
Minimum Requirements

  • Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
  • Active Illinois PT license required to start the assignment.
  • Minimum 2 years recent home health physical therapy experience with adult/geriatric patients. (Employer may consider 1 year of recent home health experience for the right candidate.)
  • Current BLS certification from AHA or ARC.
  • Valid driver's license, reliable personal vehicle, and active auto insurance. Candidate must provide transportation to all patient visits.
  • Clinical skills: ability to assist with ADLs and recommend/fit adaptive equipment. Demonstrated ability to collaborate with RN, OT, Speech and other team members.
  • Experience with EPIC EMR and OASIS documentation preferred and required by client.

Preferred Qualifications
  • Master's degree in Physical Therapy.
  • Prior experience working in a large home health census or remote/home-based practice.

Assignment Details
  • Shift start time: 8:00 AM.
  • Schedule cycle: Monday through Friday.
  • Expectation: Approximately 6 patient visits per day within the coverage area.
  • Coverage area includes Chicago ZIP codes: 60630, 60618, 60625, 60646, 60659, 60660, 60657, 60640, 60613, 60614 and surrounding areas.
  • Orientation: Approximately 2 days onsite in Oak Brook for EMR training and skills check-off, then 100% remote within coverage area. Meetings held virtually.
  • Supplies and equipment will be delivered to clinician's home.
  • Attire: solid color scrubs or business casual. No jeans or open-toe shoes.
