Key Responsibilities
  • Manage an assigned caseload of patients; evaluate their psychosocial needs through assessments and interventions aimed at dissolving barriers that affect their treatment and transition through care.
  • Implement comprehensive treatment plans that consider the diverse emotional, financial, and cultural factors that influence patient health.
  • Lead and coordinate with the healthcare team regarding protective services, life care decisions, and advance directives.
  • Provide crisis intervention and supportive counseling to patients, families, and healthcare staff during challenging situations.
  • Collaborate effectively with case managers and community resources to facilitate comprehensive education, resource allocation, and referral planning for patients and their families.
  • Educate and mentor staff, including LCSWs, MSWs, interns, and volunteers, on best practices in social work and psychosocial care.
  • Coordinate care for pediatric discharges; ensure that resources from community agencies are aligned and available to support ongoing care and prevent avoidable hospital readmissions.
  • Respond promptly to psychosocial service needs, ensuring that treatment plans are implemented without delay to facilitate timely discharge.
  • Be available for on-call duties on a rotational basis and participate in interdepartmental consultations and patient care rounds.
  • Utilize skill in navigating complex cases to advocate for patient rights and ensure their voices are heard throughout the continuum of care.
Patient Population / Client Focus

Your role will greatly impact a diverse patient population, including at-risk families, individuals dealing with chronic illnesses, and those confronting socio-economic difficulties. Qualifications
