This position is primarily remote, allowing for flexible work arrangements while remaining an integral part of our rehabilitation team. You will collaborate with physical therapists, occupational therapists, and speech-language pathologists, all working toward a unified goal of enhancing patient recovery. Although the majority of your work will be conducted from home, you may occasionally be required to participate in team meetings or training sessions at our Jacksonville facility.

• Conduct comprehensive psychosocial assessments by interviewing patients, observing their interactions, and analyzing relevant medical records to identify emotional, social, and environmental factors that may impact recovery.
• Provide compassionate counseling and support to both patients and their families, addressing concerns such as adjustments to new lifestyles, emotional distress, and coping strategies for recovery.
• Collaborate with the multidisciplinary rehabilitation team to design and implement individualized treatment plans that take into account psychosocial needs, ensuring holistic care.
• Assist patients in navigating community resources, such as support groups, vocational rehabilitation services, or housing assistance programs, empowering them to take charge of their recovery.
• Deliver educational sessions to patients and families regarding available community resources and post-rehabilitation services that can help sustain recovery post-discharge.
• Engage in discharge planning processes, facilitating smooth transitions for patients moving to home care or other care settings, confirming they have adequate support and resources available.

You will work primarily with patients in a hospice setting who are experiencing the complexities of rehabilitation due to conditions like stroke, spinal cord injury, orthopedic surgeries, and various neurological disorders. Your focus will be on offering emotional and social support that complements their physical rehabilitation, aiding them in achieving their highest level of independence and quality of life.

• A Master's degree in Social Work from an accredited program is required.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) is essential.
• Strong communication and interpersonal skills to foster relationships with patients and colleagues alike.
• In-depth understanding of psychological adjustment issues, grief processes, and coping mechanisms relevant to rehabilitation.
• A commitment to addressing and advocating for patients' emotional, social, and psychological needs during their recovery.

• Prior experience working within a rehabilitation or medical setting is preferred, allowing for a smoother integration into our multidisciplinary team.
• Experience dealing with patients facing significant life changes, trauma, or health crises will be valued.
