Job description

Work remote on a consistent PST schedule with a capped caseload-focus on patients, not paperwork.
  • Role: Care Navigator
  • Location/Type: U.S. Remote (PST hours)
  • Pay: $46,000 salary (CA: ~$25/hr)
  • Schedule: 9am-6pm PST
What you'll do
  • Make 20-70 outbound calls/day; land ~10 patient check-ins
  • Schedule visits, labs, imaging, and outside referrals
  • Coordinate care between providers and patients (clear ownership)
  • Gather medical history and document in EHR
  • Follow up after visits with next steps
  • Escalate issues to clinical team (fast feedback)
  • Keep patients engaged and supported on calls
Must-haves
  • Comfortable making high-volume outbound calls daily
  • Basic medical terminology knowledge
  • Experience speaking with patients/customers by phone
