You will have the resources and tools necessary to perform effective assessments, treatment planning, and case management from the comfort of your home, while still feeling connected to a dedicated team of professionals.

• Collaborate with a range of healthcare professionals, including physicians, nurses, and chaplains, to ensure comprehensive, patient-centered care.
• Conduct thorough psychosocial evaluations and case assessments to identify the specific needs of clients and their families.
• Develop and implement individualized treatment plans that address mental health support and palliative needs, adjusting as necessary based on client progress.
• Provide counseling and emotional support to clients and families, guiding them through the complexities of grief and loss, and equipping them with coping strategies.
• Educate patients and their loved ones about available mental health resources, including community services, support groups, and self-care strategies.
• Keep abreast of the latest trends and evidence-based practices in mental health and palliative care to inform ongoing service delivery.
• Participate in team meetings, case reviews, and interdisciplinary discussions to enhance service integration and improve client outcomes.

Our primary patient population consists of individuals with terminal illnesses and their families, facing profound emotional and psychological challenges. As an LCSW, you will play a pivotal role in not just addressing psychological well-being, but also ensuring that family dynamics are understood and supported during difficult times, laying the groundwork for healthy long-term coping mechanisms.

• Master’s Degree in Social Work from an accredited institution.
• Active Licensed Clinical Social Worker (LCSW) credential in Virginia.
• Demonstrated experience in conducting clinical evaluations and formulating treatment plans, particularly in a hospice or similar healthcare setting.
• Exceptional communication and interpersonal skills that foster collaboration and rapport building.
• Ability to navigate sensitive topics relating to end-of-life care with both empathy and professionalism.

• At least two years of experience in social work, preferably within a healthcare, hospice, or palliative care setting.
• Prior experience working with diverse patient populations and a broad understanding of culturally sensitive practices in mental health.
• Certification or specialized training in grief counseling or palliative care is a plus.
