Registered Nurse (RN) – Remote Care Management & Coordination

Job Summary: The Registered Nurse (RN) plays a critical role in delivering remote patient support, care management, and care coordination services for diverse patient populations. This position focuses on virtual engagement, chronic disease management, patient education, and proactive outreach to improve clinical outcomes and enhance the patient experience. The RN works collaboratively with multidisciplinary teams to ensure smooth care transitions, close clinical gaps, and address patient needs in a timely and compassionate manner.

Key Responsibilities:

  • Provide telephonic and virtual patient assessments, care coordination, and ongoing monitoring for assigned patient populations.
  • Deliver patient and family education related to conditions, treatment plans, medications, and self-management strategies.
  • Develop, implement, and update individualized care plans based on clinical guidelines and patient needs.
  • Monitor patient progress, escalate clinical concerns appropriately, and support early identification of deterioration or barriers to care.
