Key Responsibilities

• Assist primary care providers in identifying, treating, and managing mental health and psychosocial issues, contributing as an essential team member.
• Conduct thorough patient intakes, focusing on diagnostic and functional evaluations, and collaborate with primary care providers to establish treatment goals and plans.
• Provide brief, targeted interventions for patients in need of mental health services, ensuring timely support and follow-up.
• Identify patients requiring referral to specialty mental health facilities and initiate appropriate consultations.
• Develop and implement relapse prevention plans, helping patients maintain stability in their mental health.
• Monitor and coordinate health services delivery relating to behavioral health, linking patients with community resources as appropriate.
• Document patient progress and treatment plans in accordance with clinical standards and maintain accurate clinical records.
• Provide educational resources and self-management strategies to empower patients in their recovery journey.
• Consult with clinic management and team members regarding behavioral services and performance evaluation.
• Adhere to the Code of Ethical Conduct for Clinical Professionals and carry out additional duties as assigned.

Qualifications

Preferred Experience

• Experience with psychodiagnostics, assessment processes, and individual patient care.
• Familiarity with electronic health records (EHR) systems to document and track patient care.
• Bi-lingual proficiency in English and Spanish is preferred, as it enhances communication with a broader patient demographic.
