Key Responsibilities

• Conduct patient intakes that concentrate on thorough diagnostic and functional evaluations to inform treatment recommendations.
• Collaborate with primary care providers to recognize and manage mental health and psychosocial issues, ensuring a holistic approach to patient care.
• Provide brief but effective therapeutic interventions for patients requiring mental health services, helping them to achieve their treatment goals.
• Monitor patient progress through follow-up communications, adapting treatment plans as necessary to ensure optimal outcomes.
• Develop relapse prevention strategies for patients to maintain stable functioning and well-being.
• Identify at-risk patients and create proactive plans to prevent deterioration of their mental health conditions.
• Document patient progress and maintain accurate treatment records in compliance with agency standards and regulatory requirements.
• Advise primary care providers about referrals to specialized mental health facilities when appropriate, facilitating timely and effective care transitions.
• Provide education and resources to empower patients in self-management and recovery, fostering independence and resilience.
• Collaborate with clinic management and team members to evaluate behavioral health services and suggest improvements.

Qualifications

Compensation and Benefits

The annual salary for this position ranges from USD 87,000 to 105,000, commensurate with experience and qualifications.
