Nutritional Support Coordinator (WFH - US)
**This is a work from home position; it is not mobile.Candidates must have a private, stationary, and HIPAA-compliant workspace within their home that is dedicated solely to work. Candidates must reside in the United States to be considered.**

What We're Looking For:
We're looking for a compassionate, patient-centered Nutritional Support Coordinator to support patients' nutritional care through remote education, reinforcement, and care coordination. The ideal candidate has a healthcare background-whether as a health coach, care coordinator, medical assistant, or LPN-with experience or strong interest in nutritional support for chronic conditions. You excel at simplifying health concepts for diverse populations, know when to escalate concerns to clinical staff, and are passionate about helping patients follow through on their care plans. You thrive in fast-paced environments, build rapport easily with patients, and collaborate effectively with clinical teams.
What You'll Do:
Patient Outreach & Nutritional Counseling
  • Manage an assigned panel of 300-350 patients and conduct required monthly nutritional outreach and support.
  • Conduct 50-60+ daily phone calls with patients, providing nutritional care plan support and health coaching tailored to their specific conditions.
  • Maintain call quality standards and professional communication in every patient interaction; pass regular QA call reviews.
Nutrition Planning & Patient Education
  • Educate patients on nutrition concepts, dietary modifications, and weight management strategies.
  • Simplify complex nutritional information for diverse patient populations.
  • Reinforce provider-directed nutritional guidance and support patient adherence to established care plans.

Care Coordination & Collaboration
  • Collaborate with clinical teams, providers, and caregivers to identify and address nutritional and clinical needs.
  • Support care coordination goals by staying on schedule with monthly outreach targets.
  • Escalate patient concerns or changes in condition to appropriate clinical staff per established protocols.
Documentation & Compliance
  • Document all patient interactions in real time using the care coordination platform.
  • Maintain HIPAA compliance in all patient communications and documentation.
  • Ensure accurate documentation to support care coordination goals.

Required Qualifications:
  • Minimum 2+ years of experience in a healthcare role such as care coordination, health coaching, Medical Assistant: Must hold a nationally recognized medical assistant certification (CMA, RMA, CCMA, NCMA, CMAC, or equivalent state-issued MA-C).
  • OR Active Licensed Practical Nurse or Licensed Vocational Nurse license:
    • Must have a multi-state compact license.
    • Willing to obtain company-paid state-specific license, if needed.
  • Minimum 2+ years of clinical nutrition counseling or dietary care coordination experience.
  • 2+ years of Electronic Medical Records (EMR) and computer navigation experience.
  • Demonstrated ability to educate patients and simplify complex nutritional concepts.
  • HIPAA-compliant home office setup with minimum 50 Mbps download / 5 Mbps upload internet capability.
  • Must be legally authorized to work in the United States.

Preferred Qualifications:
  • 3+ years of experience in healthcare nutrition, chronic care management, or patient education.
  • Certification as a:
    • Registered Dietetic Technician (NDTR), or
    • Certified Nutritionist, or
    • Certified Health Coach
  • Bilingual (Spanish/English) preferred.
  • Experience in remote patient care or telehealth nutrition counseling.
  • Experience with care management or chronic disease management programs.
  • Familiarity with weight management or behavioral health principles.
  • Previous experience managing large patient panels (200+).
  • Bilingual capability (English and Spanish preferred).
