JOB TITLE: On-Demand Nurse Practitioner - Preventative Care
JOB TYPE: Contract (1099) | WEEKEND/HOLIDAY SHIFTS ONLY

REPORTING TO: Head of Strategic Initiatives
JOB SCOPE:
We are seeking a dedicated and experienced Michigan-licensed Nurse Practitioner to join our team for an on-demand role. This position involves contacting patients with abnormal results from screening events and providing them with next steps and guidelines. The role is structured around scheduled events occurring on a weekend shift, with a minimum of two (2) Sundays per month. Routine shifts tend to be 6-8 hours.
MAJOR JOB DUTIES:

  • Contact patients with abnormal screening results to discuss findings and provide guidance on next steps.
  • Deliver clear and concise information about the nature of the abnormalities and potential health implications.
  • Develop and communicate personalized care plans, including necessary follow-up actions and preventive measures.
  • Collaborate with a multidisciplinary team to ensure comprehensive patient care.
  • Maintain accurate and timely documentation of patient interactions and care plans.
QUALIFICATIONS:
  • Active Nurse Practitioner license in the state of Michigan.
  • Minimum of 2 years of clinical experience, preferably in a preventative care setting.
  • Strong communication and interpersonal skills.
  • Ability to deliver sensitive information with empathy and professionalism.
  • Proficient in electronic health records (EHR) systems.
