• Conduct thorough clinical assessments, utilizing standardized tools to assess the mental and emotional needs of patients and families dealing with end-of-life issues.
• Develop tailored treatment plans integrating evidence-based therapeutic interventions such as cognitive behavioral therapy and solution-focused brief therapy.
• Maintain comprehensive and accurate clinical documentation, ensuring compliance with all regulatory and organizational requirements.
• Collaborate closely with physicians, nurses, and other healthcare professionals to provide coordinated care, facilitating regular case discussions and treatment plan reviews.
• Provide ongoing education and emotional support to patients and their families, equipping them with coping strategies and resources during a challenging time.
• Stay updated on the latest mental health research and therapeutic interventions to continuously improve your clinical practice and patient outcomes.

In this role, you will be primarily serving patients facing terminal illnesses and their families. Your expertise will aid in addressing complex emotional and psychosocial issues, thereby facilitating a holistic approach to care that enhances the quality of life during the final stages of life. Your role in managing grief, loss, and anticipatory bereavement is crucial in creating a supportive environment that fosters resilience and healing.

• A Master’s degree in Social Work (MSW) from an accredited institution.
• A valid and active MSW license in the state of Michigan.
• Proficient knowledge in a variety of psychotherapy interventions, particularly those responsive to the unique challenges faced by end-of-life patients.
• Exceptional communication and problem-solving skills, with the ability to engage empathetically with diverse patient populations and their families.

• A minimum of 2 years of direct clinical experience, ideally in a hospice, palliative care, or mental health setting.
• Experience with telehealth services and electronic health record systems, facilitating efficient patient care and communication.
• Familiarity with local resources and community support services available to patients and families navigating end-of-life care.
