Job description

Puget Sound Home Health & Hospice is seeking an experienced Part-time MSW or LICSW to join our home health team in King County (Kent, Burien, Renton, etc.).

Home Health Social Worker responsibilities:

  • Assess the psychosocial status of patients related to patient’s illness

  • Develop and provide multiple community and governmental resources to support a patient in their home environment

  • Provides assessment in patient’s residence and carries out social evaluations and plan interventions

  • Participate in the development of the individualized plan of care

  • Assist physician in understanding significant social and emotional factors

  • Participate in quality assessment performance improvement team

  • Serve as liaison between patients and community agencies

  • Maintain and develop contracts with public and private agencies

Qualifications:

  • A graduate of an accredited master’s program in social work

  • Licensed MSW or LICSW in Washington State

  • Minimum of one year’s experience in health care

  • Experience with home health or hospice preferred

  • Possesses and maintains current BLS Certification

  • A licensed driver with an insured automobile
