About the Role

As a Clinical Pharmacist with PharmD Live, you'll engage directly with patients to deliver proactive, pharmacist-led clinical care that improves outcomes and drives value-based success. You'll provide services such as Chronic Care Management (CCM), Transitions of Care Management (TCM),
Remote Patient Monitoring (RPM), and Medication Therapy Management (MTM) through secure telehealth platforms, ensuring each patient receives personalized, evidence-based support.


Key Responsibilities

  • Deliver disease and medication management services via phone or telehealth (video).
  • Conduct comprehensive medication reviews to identify therapy gaps and medication-related issues.
  • Monitor drug therapy for contraindications, drug-drug or food interactions, and dosing accuracy.
  • Recommend cost-effective, clinically appropriate alternatives to medical staff.
  • Collaborate closely with internal care teams and external partners to coordinate patient care.
  • Document all clinical and care management activities in PharmD Live's proprietary system.
  • Educate patients on medication use, adherence strategies, and treatment goals.
  • Contribute to quality initiatives by sharing insights and supporting program enhancements.
  • Maintain professional growth through ongoing education and research.
  • Provide clinical oversight and support to ensure timely and value-driven client delivery.
  • Present updates and progress reports to leadership as needed.

Qualifications & Skills

  • PharmD from an ACPE-accredited College of Pharmacy.
  • Active Minnesota pharmacist license or eligibility for licensure.
  • Board Certification (BCGP, BCPS, BCACP) or willingness to obtain within 12 months.
  • Demonstrated independence and sound clinical judgment.
  • Current knowledge of all phases of pharmacy practice.
  • Strong communication, documentation, and analytical skills.
  • Ability to manage multiple priorities in a dynamic care environment.
  • Proficiency with Google Workspace and Windows platforms.
  • Empathetic, patient-centered, and collaborative approach to care delivery.
