Job description

Job Overview
Fairview is seeking an experienced Registered Nurse (RN) Navigator to support our Gastroenterology (GI) team in a 100% remote role. This position is approved for 0.8 FTE (64 hours per pay period) and works Monday through Friday, 8:00 a.m. to 4:30 p.m., with one scheduled day off per week. There are no weekend, holiday, or on-call requirements.
Provides care navigation, communication and education to patients and family members through triaging of patients after a referral to clinic is placed. Works closely with providers and scheduling team to best meet the needs of patients. Corresponds with referring providers, internal and external as needed. Open lines of communication between RN navigator and patient from referral until initial consultation with scheduling team. They anticipate patient needs and take appropriate courses of action using approved guidelines. Excellent problem solving and communication skills. Helps patient and care team navigate additional work up or testing needed prior to consultation.
Responsibilities
  • Navigates patient care from initial referral until consultation with care team.
  • Coordinates care for patients new to the specialty.
  • Works with patients and families to prioritize patient needs and goals for initial appointments.
  • Anticipates future needs of patient and educates and/or refers to appropriate sources.
  • Assesses patient clinical history and, if known, diagnosis, to create an initial plan for provider appointments and, if appropriate, further diagnostic tests.
  • Collaborates with schedulers to schedule initial appointment(s) and assists scheduling team to resolve any barriers to carry out appointment plan.
  • Provides patient/family education regarding additional testing and work up which is needed.
  • Develops and engages patient and family in plan of care.
  • Works at highest level of scope of practice and skills.
  • Documents and updates EHR as initial appointment care plan is developed.
  • Follows standard work guides and workflows.
  • Collaborates with records team to obtain notes, imaging, results, etc... from outside the health system.
  • Serves as resource to referring offices regarding specialty.
  • Maintains strong communication and relationship with clinic team and referring office
  • Communicates with referring providers to gain additional clinical history and/or inform of the initial appointment plan. Also communicates recommendations based on provider review of patient.

Required Qualifications
  • Associates Degree in Nursing or
  • Bachelors Degree in Nursing
  • 3 years nursing experience within specific patient population group
  • MN Registered Nurse (RN) License
