Job Description:
Job Summary
Responsible for obtaining accurate home medication lists from patients entering the hospital. Assists pharmacy, nursing, and medical staff by conveying medication information verbally or electronically to appropriate care providers. Ensures accuracy and completeness of medication histories in various hospital settings. Collaborates with multidisciplinary teams to support safe patient care transitions. Maintains compliance with organizational procedures and quality standards.
Essential Functions
• Conducts interviews and phone calls to retail pharmacies and physician offices to gather home medication histories.
• Utilizes medication history software and state databases to obtain medication information.
• Updates computerized home medication databases and reconciles medication lists with admission orders.
• Discusses high-alert or targeted medications with the pharmacist as appropriate.
• Notifies the pharmacist immediately of discrepancies between updated home medication lists and admission orders.
• Collects data and conducts quality monitors as assigned.
• Completes required documentation accurately and timely.
• Performs other duties as assigned.
Education Requirement
High school diploma or equivalent and vocational/technical training in a designated technical area
Experience Requirement
0-2 years of experience in a healthcare, clinical setting, or a relevant area
An equivalent combination of education and experience may be considered.
All experience must be paid and in the same related field.
Part-time and PRN experience will be prorated based on hours worked per week.
Volunteer work and internships for academic credit are not counted.
Certifications & Licensures
Active KY Board of Pharmacy Registration
Nationally certified Pharmacy Technician by either PTCB (PTCE) or NHA (ExCPT)
