This position is designed for a remote work setting, allowing flexibility in managing your time while ensuring high-quality care to your clients. Although based remotely, you will be part of an interdisciplinary team that includes healthcare providers and caregivers, facilitating collaboration and communication to address client needs effectively. You are expected to work a full-time schedule from Monday to Friday, typically from 8:00 AM to 4:30 PM, with occasional evening or weekend hours when necessary to accommodate client availability.

• Conduct thorough assessments of each client's psychosocial needs, considering emotional, mental, and social factors affecting their well-being.
• Develop personalized care plans in collaboration with seniors and their families, ensuring alignment with their preferences and goals.
• Provide counseling and emotional support tailored to help seniors navigate life transitions, including health changes and family dynamics.
• Advocate for clients, ensuring they have access to necessary resources and support services within the community.
• Coordinate with an interdisciplinary team, including nurses and healthcare professionals, to enhance the overall plan of care for each client.
• Conduct regular check-ins to monitor clients' well-being and address any emerging concerns regarding their safety and living conditions.
• Facilitate effective communication among family members, resolving conflicts and providing mediation when necessary.
• Educate clients and their families on available community resources and encourage participation in social programs to promote engagement.
• Maintain precise and updated documentation of client assessments, care plans, and all interactions to comply with regulatory standards.
• Stay informed about the latest practices in social work, community resources, and changes in policies affecting your clients.

This role specifically targets the senior population, focusing on their distinct challenges and opportunities. You will work closely with seniors who may be contending with issues such as isolation, grief, health-related transitions, or family disputes. Your empathetic approach will empower these clients by ensuring they feel seen, heard, and understood in their journeys.

• A Master's degree in Social Work from an accredited institution is necessary to establish a comprehensive understanding of clinical standards and practices.
• Active and valid licensure as an LCSW, confirming your credentials and ability to practice in the state of Texas.
• A minimum of two years of hands-on experience in social work, particularly within community settings or with geriatric populations, is preferred.
• Thorough understanding of social work principles, aging-related issues, and the resources available in the community.
• Exceptional skills in assessment, counseling, and team-based problem-solving are essential for fostering collaborative care.
• Ability to build strong rapport with elderly clients and families to ensure trust and facilitate open communication.

• Proficiency in electronic health record (EHR) systems is advantageous as you'll be documenting client interactions routinely.

• Experience working in a community health clinic or similar setting that specializes in serving seniors is preferable.

The salary for this role ranges from USD 60,000 to 100,000 annually, based on experience and qualifications.
