• Conduct in-depth clinical assessments, identifying patient needs and developing personalized treatment plans that integrate solution-focused approaches.

• Implement evidence-based therapeutic interventions, focusing on the emotional and psychological well-being of patients, while closely monitoring progress towards treatment goals.

• Document clinical encounters accurately and promptly, maintaining compliance with ethical guidelines and organization policies.

• Collaborate with a diverse team of healthcare providers, including physicians, nurses, and chaplains, to facilitate holistic and coordinated care for patients.

• Participate in case consultations and peer review meetings that aim to elevate the quality of care through shared insights and best practices.

• Provide education and support to patients and families, equipping them with valuable resources and insights to navigate mental health challenges in a hospice context.

• Stay informed on current research and best practices within the field of social work, integrating new knowledge into your practice to enhance patient outcomes.

In this role, you will primarily serve patients facing terminal illnesses and their families, offering emotional support and practical assistance in coping with grief, loss, and life transitions. Your expertise will significantly contribute to their well-being by addressing various psychosocial issues and ensuring that they receive compassionate, empathetic support during their journey.

• A Master’s degree in Social Work from an accredited institution is required.

• Possession of a valid LCSW license in the state of Georgia.

• Minimum of 2 years' experience in clinical social work, particularly in hospice or palliative care settings.

• Proficiency in utilizing solution-focused therapy and other evidence-based psychotherapy interventions.

• Strong communication and interpersonal skills that foster collaboration and building rapport with patients and families.

• Ability to thrive in a virtual team environment and remain organized in managing a diverse caseload.

• Experience specifically within hospice care or end-of-life counseling is highly advantageous.

• Demonstrated success in managing therapeutic relationships and delivering high-quality mental health services under varying circumstances.
