This position is uniquely designed for a remote work setting, allowing you to provide support from the comfort of your own home while maintaining flexibility. Despite the remote nature of this role, you will be an integral part of our hospice team, utilizing digital communication tools and electronic medical records systems to connect with colleagues and monitor patient progress.

• Collaborate with healthcare team members to create individualized treatment plans based on detailed psychosocial assessments and accepted social work methodologies.
• Conduct comprehensive assessments for patients that identify emotional, behavioral, and social challenges and implement necessary interventions.
• Maintain accurate and timely clinical documentation, including progress notes, in adherence to department policies, ensuring all records reflect the patient's current status and care provided.
• Provide ongoing counseling and crisis intervention services to patients and families, addressing urgent emotional needs and guiding them through challenges related to end-of-life care.
• Engage with family members and support systems, effectively coordinating discharge plans and ensuring a smooth transition to home care or other services.

The ideal candidate will work primarily with clients residing in hospice care, a sensitive and vital area where emotional well-being is paramount. You will support individuals facing terminal illnesses, helping them and their families navigate complex feelings concerning grief, loss, and end-of-life decisions.

• A Master’s degree in Social Work from an accredited program is mandatory; a focus on clinical practice or psychosocial intervention is desirable.
• An active state license as a Licensed Clinical Social Worker (LCSW) is required to ensure compliance with professional standards.
• Strong proficiency in computer applications, including electronic medical records (EMR) systems, word processing, and spreadsheet management.
• A diligent work ethic is essential, along with the ability to effectively prioritize tasks in a dynamic healthcare environment, adapting to changing needs and situations.

• At least 2-3 years of experience in clinical social work, ideally within hospice or palliative care settings, is strongly preferred to provide the depth of understanding necessary for success in this role.
• Experience with crisis intervention, grief counseling, and family dynamics related to end-of-life issues will be essential.
