This position is designed to operate primarily in a remote setting, allowing for flexibility while still engaging directly with clients via teletherapy platforms. Our outpatient office serves as a central hub for patients seeking various healthcare services, making it essential for our counselors to maintain clear communication with other healthcare providers. • Collaborate with primary care providers to recognize, address, and manage mental health and psychosocial issues effectively.
• Conduct comprehensive patient intakes, focusing on diagnostic assessments and functional evaluations, and provide actionable treatment recommendations.
• Offer brief, focused therapeutic interventions tailored to patients' mental health needs.
• Advise and consult with primary care teams regarding patient referrals to specialty mental health services when necessary.
• Initiate follow-up communications to monitor patient progress, modifying treatment plans as needed to ensure optimal care.
• Develop personalized relapse prevention plans and assist patients in achieving stable functioning in their daily lives.
• Identify “at risk” patients and implement strategies to preempt the deterioration of their mental health conditions.
• Document patient progress and maintain treatment records as required by agency guidelines.
• Engage in proactive communication with primary care providers to keep them informed about patient needs and treatment developments.
• Empower patients by providing them with self-management strategies and education to enhance their role in treatment processes.

The Licensed Professional Counselor will work with a diverse patient population, addressing a broad spectrum of mental health issues across various age groups. Our clients come from varying socioeconomic backgrounds and present with unique challenges, including anxiety, depression, trauma-related disorders, and other mental health conditions. The ideal candidate must possess the skills to foster trusting relationships, ensuring that all patients feel supported and valued within a therapeutic relationship.

• A minimum of a Master's Degree in Clinical Counseling or a related field is required.
• Current Licensed Professional Counselor (LPC) license is mandatory.
• Solid understanding of mental health assessment techniques, treatment planning, and evidence-based psychiatric therapies.
• Familiarity with DSM-IV diagnoses and various psychotherapeutic interventions.
• Capacity to tailor approaches based on individual patient needs and community resource availability.

• Preference for bilingual candidates, specifically those proficient in both English and Spanish.

For this full-time position, the compensation ranges from USD 87,000 to 105,000 annually, commensurate with experience.
