We're Looking For

We are seeking a dedicated and experienced Registered Nurse (RN)to join our team, full-time. Ideal nursing candidates demonstrate a strong commitment to service excellence, nursing excellence, and adaptability.

Requirements

Completion of an accredited or approved program in nursing

Registered Nurse license issued by the state in which the teammate practices

3 years of nursing experience preferred

BSN preferred

Prior nurse navigation experience preferred

Ambulatory, triage, or care coordination/navigation experience preferred

Position Details:

Remote Monday - 8:00a to 5:00p no weekends or holidays

We are seeking an RN to support outreach, care coordination, and scheduling for its Transition Clinic locations.This is a fully remote, virtual role.The RN will be responsible for receiving and reviewing referrals following recent patient discharges, determining appropriate next steps, and scheduling patients with a provider or connecting them to the appropriate care team as needed.

What You'll Do:

As a skilled and compassionate RN, you will play a pivotal role in providing and coordinating comprehensive patient care through the nursing process to deliver safe, therapeutic care in accordance with established standards, policies, and procedures.

Using evidence-based practice, clinical decision making, compassion, and skills communication while leading efforts to create the safest patient environment and the best patient experience across the continuum.
