Weekend Nurse (LPN/LVN) - AL, GA, KY, MS, or TN licensure required

Position Overview:
As a TeleMate Health part-time Weekend Nurse, you will play a vital role in delivering patient-centered care. You will play a key role in providing quality patient care through oversight, education, and communication with the patient's existing healthcare team. This position offers the opportunity to make a meaningful impact on patients' lives.


Hours:

A Weekend Nurse's hours are 4:30pm CST - 8:30pm CST on Friday evenings and 8:00am CST - 4:30pm CST on Saturdays and Sundays


Key Responsibilities

  • Provide oversight on patient's vital signs and taking appropriate action when readings are outside of parameters
  • Being a patient advocate, meeting them where they are in their individual healthcare journey
  • Conduct a thorough, holistic assessment of patients' health conditions and needs
  • Educate patients and caregivers on managing disease processes and medications
  • Educate patients on utilizing RPM technology effectively and efficiently
  • Provide verbal reports to physician offices and help bridge the gap between patient and healthcare provider
  • Effectively provide skilled documentation related to patient communication and education
  • Assess for acute exacerbations when necessary
  • Ability to work a minimum of five days a month and open to assisting during holidays


Qualifications and Skills

  • Graduated from an accredited program
  • Current licensure and accreditation for Licensed Practical Nurse (LPN), or Licensed Vocational Nurse (LVN)
  • Strong communication skills and a commitment to providing compassionate care.
  • Ability to work independently and manage time effectively in a remote environment.
  • Proficiency with electronic health record (EHR) systems and telehealth technologies.
  • High speed internet at home
  • Private and quiet workspace at home where phone conversations are uninterrupted and PHI cannot be overheard.
  • Strong desire to provide patient advocacy
  • Previous experience in telehealth or virtual care settings is a plus.
  • Ability to multitask
  • Critical thinking skills for assessing and addressing patient needs
  • Ability to adapt conversations based on patient need and knowledge base
  • Compassion for the geriatric population, empathetic, and intuitive listening skills
  • Fluency in English is required, additional languages are a bonus
  • Physically reside in the U.S. and be legally eligible to work for any employer
  • Must pass background check
