In a remote setting, Licensed Professional Counselors at Yakima Family Health Services will engage with clients through virtual platforms, allowing for flexible hours tailored to appointment schedules. • Conduct comprehensive patient intakes, including diagnostic and functional evaluations, to inform treatment goals and plans.
• Provide brief, focused interventions to patients requiring mental health support and evaluate their treatment needs consistently.
• Collaborate with primary care providers by identifying patients at risk for further mental health issues and determining appropriate levels of care and referral when necessary.
• Monitor patient progress through regular follow-up communications and adjust treatment strategies in accordance with clinical best practices.
• Develop and implement relapse prevention plans for patients, ensuring ongoing support for stable functioning in their daily lives.
• Track and document patient outcomes and treatment progress accurately within electronic health records to maintain compliance with agency and Federally Qualified Health Center (FQHC) protocols.
• Work closely with patients to equip them with self-management skills and educational resources necessary for their engagement in therapeutic processes.
• Consult with clinic management and team members to foster improvement in behavioral health services and assist with program evaluation efforts.
• Adhere to the ethical guidelines set forth in the Code of Ethical Conduct for Clinical Professionals while performing all duties.

As a Licensed Professional Counselor at Yakima Family Health Services, you will primarily work with diverse populations from the local community, including individuals from various socio-economic backgrounds and age groups. Your role will be crucial in addressing a spectrum of mental health concerns, ensuring that each patient receives tailored support that respects their unique circumstances and facilitates their wellness journey.

• A Master's Degree in Clinical Counseling or a relevant field is required.
• A current LPC (Licensed Professional Counselor) License is a must for this position.
• Thorough understanding of mental health assessments, treatment planning, and various therapeutic modalities.
• Strong interpersonal skills and the ability to establish rapport with patients, their families, and other health professionals.
• Ability to work effectively within an interdisciplinary team and communicate ideas clearly.

• Experience in community health settings or Federally Qualified Health Centers (FQHC).
• Familiarity with DSM-IV diagnoses and behavioral therapy techniques.
• Bilingual proficiency in English and Spanish is preferred, though not required.
