• Conduct comprehensive psychosocial assessments of patients using standardized tools and methodologies to identify their emotional, social, and financial needs.
• Collaborate actively with medical teams to develop holistic care plans that address both medical and psychosocial concerns, ensuring a well-rounded approach to each case.
• Offer individual and group counseling services, providing specialized support in areas such as grief, adjustment issues, and coping strategies tailored to each patient's circumstances.
• Coordinate post-discharge services by partnering with community organizations to ensure patients have continued access to necessary resources and facilities.
• Deliver emotional support and educational resources to patients and their families, helping them navigate treatment options and emotional challenges gently and empathetically.
• Engage in crisis intervention and provide timely emotional support during urgent situations, ensuring patient safety and well-being.
• Document care activities accurately and timely, ensuring compliance with hospital policies and legal requirements, thereby maintaining the integrity of patient records.
• Participate in hospital committees, contributing social work expertise to initiatives that enhance service delivery and patient outcomes.

Your work will focus on diverse patient populations, including individuals facing mental health challenges related to physical illnesses, trauma, and socioeconomic factors. You will address clients from various backgrounds and age groups, ensuring that care is culturally sensitive and tailored to each individual’s unique situation. By understanding the specific needs and concerns of our clients, you will help facilitate their journey towards better mental health.

• A Master's degree in Social Work from an accredited institution is required.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) is essential for this role.
• A minimum of two years of experience in a healthcare setting is ideal, though exceptional new graduates will also be considered.
• Proficiency in medical terminology, mental health disorders, and crisis intervention techniques is highly valued.
• Exceptional communication, interpersonal, and collaborative skills to effectively liaise with a variety of stakeholders.

• Experience in providing counseling within a community health clinic or similar environment is preferred.
• Familiarity with electronic health record systems to facilitate efficient documentation and inter-departmental communication.

The salary for this position ranges from USD 70,000 to 100,000 annually, contingent on experience and qualifications.
