• Conduct comprehensive assessments of clients to identify mental health issues and determine appropriate intervention strategies.
• Develop and implement individualized treatment plans that reflect the unique circumstances and needs of each client.
• Provide evidence-based individual, couples, and family therapy sessions, utilizing established therapeutic modalities tailored to hospice care.
• Collaborate with physicians, nurses, and other healthcare professionals to ensure a holistic approach to patient care, sharing insights and progress during care team meetings.
• Maintain accurate and timely documentation of client encounters, progress notes, and treatment plans in accordance with HIPAA regulations.
• Engage in ongoing supervision, attend workshops, and participate in professional development activities to enhance clinical skills and stay current with best practices.
• Adhere to all legal, ethical, and professional standards related to social work practice.

Your primary focus will be on clients in hospice care. These individuals may be dealing with terminal illnesses, requiring sensitive and empathetic interventions to help them cope with grief, anxiety, depression, and end-of-life issues. You will also work with their families, providing them with support and resources during this challenging time, ensuring a compassionate and understanding environment.

• Master's Degree in Social Work from an accredited institution.
• Current active state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of three years’ experience providing mental health services in a clinical or outpatient setting, with specific experience in hospice care preferred.
• Solid understanding of therapeutic modalities and evidence-based approaches relevant to individual and family therapy.
• Exceptional communication, interpersonal, and assessment skills that promote rapport and trust with clients.

• Experience working in a hospice or palliative care environment.
• Familiarity with the unique challenges faced by patients with terminal illnesses and their families.
• Knowledge of community resources and support services to aid clients and their families.
