The Licensed Clinical Social Worker will assume numerous responsibilities critical to the organization’s mission, including:

• Establishing therapeutic rapport with clients and their families through compassionate communication and trust-building techniques.
• Conducting comprehensive assessments, including mental health evaluations and risk assessments, tailored to the unique needs of clients in hospice care.
• Developing and implementing individualized care plans in collaboration with healthcare teams, ensuring a seamless exchange of information among involved parties.
• Providing crisis intervention and emotional support during acute episodes, actively listening to the needs and concerns of patients and their families.
• Maintaining up-to-date and accurate client records and documentation, adhering to healthcare regulations and organizational standards.
• Participating in interdisciplinary team meetings to optimize patient outcomes and to consistently improve quality of care delivery.
• Engaging in community outreach and support initiatives to educate families on available resources and coping strategies.
• Contributing towards departmental performance improvement projects and quality initiatives that enhance service delivery and patient care.

This role centers on serving clients and families within the hospice demographic, who often face complex emotional and psychological challenges related to end-of-life issues. The social worker will engage with individuals experiencing major mental illnesses, severe emotional disorders, and other psychosocial concerns to ensure that both patients and their families receive the appropriate support and care.

To be considered for this position, candidates must possess:

• A Bachelor’s degree in Social Work (BSW) or related field, with a preference for candidates holding a Master’s degree in Social Work (MSW).
• State licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of 2 years of relevant clinical experience, particularly in mental health or hospice settings.
• Strong communication skills, both verbal and written, allowing for effective interactions with clients, families, and staff.
• Solid organizational and planning capabilities, coupled with the ability to adapt to changing circumstances and client needs.

While not mandatory, bilingual proficiency in English and Spanish is highly preferred to better serve a diverse client population. Additionally, familiarity with electronic health record systems and telehealth platforms will be an asset.
