• Conduct thorough psychosocial assessments to identify clients' mental health issues, strengths, and resource needs.

• Design and implement individualized treatment plans, incorporating measurable goals and appropriate interventions for each client.
• Collaborate with treatment teams to ensure coordinated, holistic care for clients, adapting strategies as necessary based on client feedback and progress.
• Provide crisis intervention services, assessing risk and creating safety plans when immediate challenges arise.
• Maintain accurate, timely documentation of client interactions in line with ethical and regulatory standards.
• Facilitate group therapy sessions, psychoeducation workshops, and support groups, fostering a community of healing among clients.

Your work will primarily involve patients receiving hospice care, who may be experiencing complex emotional and psychological challenges as they navigate end-of-life issues. By focusing on this sensitive population, you will play a vital role in addressing their mental health needs, helping clients cope with grief, loss, and the emotional turmoil often associated with terminal illnesses.

• Master's degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) in Arizona.
• Demonstrated knowledge of various evidence-based therapeutic approaches and treatment methodologies relevant to mental health practice.
• Exceptional interpersonal, communication, and assessment capabilities, enabling effective engagement with clients and their families.

A minimum of two years of clinical experience in mental health or outpatient settings is preferred, particularly in roles focused on grief counseling or dealing with terminal illnesses. The salary for this position ranges from $80,000 to $100,000 annually, dependent upon experience and qualifications.
