• Collaborate closely with physicians, nurses, and other interdisciplinary team members to ensure coordinated and holistic care, participating actively in care conferences to refine treatment approaches.
• Advocate for patients' rights, ensuring they and their families have access to relevant community resources and support services.
• Maintain accurate records of client interactions, treatment plans, and outcomes through secure electronic health record systems, ensuring compliance with state and federal regulations.
• Engage in regular supervision sessions and continuing education opportunities to enhance your clinical skills and stay abreast of the latest in social work best practices, particularly in hospice and palliative care.

• A Master’s degree in Social Work from an accredited institution.
• A current certification as a Licensed Clinical Social Worker (LCSW) in the state of California is required.
• A minimum of two years post-licensure experience in clinical social work, preferably within hospice or palliative care environments.
• Exceptional skills in conducting diagnostic assessments, developing individualized treatment plans, and utilizing therapeutic practices in a remote setting.

The role offers a competitive annual salary in the range of USD 115,000 to 130,000, commensurate with experience and caseload.
