In your role as an LCSW, you will be tasked with the following responsibilities:

• Conducting thorough clinical assessments, including psychosocial evaluations, to identify the mental health needs of patients and their families.
• Creating and implementing individualized treatment plans that encompass solution-based interventions tailored to each client’s unique circumstances.
• Providing therapeutic services using various modalities such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), ensuring approaches align with patient preferences.
• Maintaining meticulous clinical documentation in compliance with HIPAA regulations, ensuring patient confidentiality and data security.
• Engaging in interdisciplinary collaboration, working closely with psychiatrists, psychologists, and nursing staff to provide coherent and holistic care.
• Leading case consultations and participating in peer review meetings, contributing to a culture of collective learning and professional development.
• Educating patients and their families about mental health conditions, treatment methodologies, and community resources that facilitate their journey toward better health.
• Staying abreast of developments in the mental health field and integrating new research findings into your clinical practice.

Your primary focus will be on patients in hospice care, who are often dealing with complex emotions related to terminal diagnoses. This role requires a sensitive approach to grief, loss, and the mental health issues that accompany end-of-life care. You will also support families in navigating these challenging experiences, providing them with the resources and coping strategies necessary to manage their emotional well-being.

• A Master's degree in Social Work from an accredited institution.
• A valid and active license to practice as a Licensed Clinical Social Worker (LCSW) in California.
• A minimum of 3 years of experience delivering psychotherapy services in clinical settings, particularly with hospice patients or in palliative care.
• Comprehensive understanding of solution-focused therapy techniques and their application in clinical practice.
• Outstanding communication and interpersonal skills, enabling effective collaboration and support for patients and families.
• Strong analytical, problem-solving, and critical thinking skills to address complex clinical cases.
• The ability to work fluently within a team setting while also being adaptable to individual work scenarios.

While not mandatory, preference will be given to candidates with:

• Experience in grief counseling and chronic illness management.
• Familiarity with telehealth systems and conducting remote therapy sessions effectively.
• Continued education or certifications in advanced therapeutic techniques relevant to hospice care.

The salary range for this position is between $115,000 and $130,000 annually, commensurate with experience and qualifications. In addition, we offer:
