This position is entirely remote, allowing you to work from the comfort of your home in Oceanside, California, while actively contributing to a dynamic hospice center environment. You will have the opportunity to collaborate with a diverse team of healthcare professionals, including case managers, nurses, and other social workers, leveraging technology for seamless communication and service coordination.

• Conduct thorough assessments of mental health needs for hospice patients, creating and implementing individualized treatment plans in accordance with best practices.
• Maintain ongoing documentation, including intake assessments, progress notes, and treatment plan revisions, ensuring compliance with company policies and state regulations.
• Provide direct clinical services, employing evidence-based modalities such as Cognitive Behavioral Therapy (CBT), Acceptance and Commitment Therapy (ACT), and Motivational Interviewing (MI), tailored to the specific needs of individuals, couples, and families.
• Engage in collaborative discussions with families and healthcare providers to coordinate holistic care plans that address both medical and emotional needs.
• Commit to completing a minimum of 25 client-facing visits weekly while ensuring optimal work-life balance.
• Participate actively in clinical meetings, training sessions, and community outreach efforts, contributing to the ongoing development of the healthcare team.
• Respond to and communicate effectively with requests and feedback from the Clinical Director in a timely and professional manner.
• Engender a supportive team atmosphere that respects autonomy, promotes personal growth, and nurtures creativity in problem-solving and client interventions.

This role primarily supports patients in hospice care, addressing both the mental and emotional challenges faced during end-of-life transitions. • A Master’s degree in Social Work, Counseling, Psychology, or a related mental health discipline is required.
• Active and independent licensure as a Licensed Clinical Social Worker (LCSW) in California is mandatory.
• Demonstrated ability to set and maintain appropriate boundaries while fostering therapeutic relationships with clients.
• Proficient with electronic health record (EHR) systems and Microsoft Office 365 for efficient documentation and communication.
• Strong written and verbal communication skills, crucial for effective collaboration and documentation.

• Experience working in hospice, palliative care, or with seriously ill populations is a significant advantage.
• Familiarity with interdisciplinary team collaboration and community-based mental health interventions.
• Previous experience developing SMART treatment plans and completing comprehensive clinical documentation.

We offer a competitive salary range of $120,000 to $130,000 yearly, commensurate with experience and qualifications.
